Middletown Recreation Financial Aid Information


Our financial aid program is available to City of Middletown families only. Eligibility is based on total household income, family size, financial need or hardship.

You must register yourself only, fill out additional forms and provide some combination of the documentation listed below to be reviewed for financial aid eligibility.

Proper documentation must be submitted in order to be considered for eligibility. In order to process your application, you MUST provide copies of the following items:

1. Photo ID
2. Proof of Middletown residency
3. Copy of all participant's birth certificates

Other documents that may be required:

1. Copy of most recent tax return for everyone living in the household or verification of non-requirement to file
2. The last two paycheck stubs of everyone in the household who is working
3. Care 4 Kids PPA
4. A letter from the Social Security Administration
5. Proof of any other source of income
6. State Budget Sheet
7. A Letter / e-mail from your principal, social worker, or case manager confirming hardship.
